Repeat Medications Request

Use this form to request repeat medications for your pet.  Please select your local GVG clinic when submitting the completed form.

Please note: Our veterinarians are bound by Victoria's Department of Health regulations and the Veterinary Practice Act 1997 (Vic) when prescribing medications. We are required to keep accurate records of all medications dispensed and to ensure that ongoing prescriptions remain appropriate.  As a result, the following criteria apply:

• Your pet must have been assessed by a GVG veterinarian within the past six months.  If not, an appointment may be required before further medication can be dispensed. 
• Some medications require more frequent check-ups or monitoring
• Some medications require bloods tests at specific intervals
• We can only prescribe or dispense limited quantities of medication at one time
• Each request must be reviewed and authorised by a veterinarian, so please allow up to two business days for preparation
